Adult Long-Term Care Waiver Programs


Adult long-term care (LTC) waiver programs refers to Family Care, Family Care Partnership, Program of All-Inclusive Care for the Elderly (PACE), and IRIS (Include, Respect, I Self-Direct). Adult LTC waiver programs provide any service or support that people may need because of a disability, getting older, or a chronic illness that limits their ability to do the things that are part of their daily routine. This includes things such as bathing, getting dressed, making meals, going to work, and paying bills. Click here to see what's new!

IRIS (Include, Respect, I Self-Direct)

These programs are a comprehensive and flexible long-term care service system, promoting independence and quality of life, while recognizing the need for support.

IRIS is a fully self-directed Medicaid program for Wisconsin's frail elders and adults with disabilities built on the principles of self-determination and self-direction.




What's New?
A summary of what is new for adult long-term care waiver programs: Providers who serve members or participants in Family Care, Family Care Partnership, PACE, and IRIS must enroll as Medicaid providers through ForwardHealth. For more information, visit the Provider Enrollment Information page.
