Children’s Long-Term Support Program


The Children's Long-Term Support (CLTS) Program provides services for children and young adults under the age of 22 with significant developmental, physical, or emotional disabilities. These services support participants and their families to remain in their home or community.

For information specific to children and families, visit the Wisconsin Department of Health Services (DHS) site.

DHS has contracted with a third-party administrator (TPA) claims processing vendor for the CLTS Waiver provider services. The DHS TPA vendor is Wisconsin Physicians Service (WPS). WPS processes and issues payments for CLTS services, collects and reports claims data, and recovers overpayments on behalf of DHS according to federal regulations.

Wisconsin Physicians Service Provider Portal
The Wisconsin Physicians Service (WPS) provider portal is available for CLTS providers to obtain access to participant information, authorizations, claims, and secure messaging.
Waiver Agency Portal
The waiver agency portal is available for county waiver agencies to access interChange functionality, enter/update CLTS enrollment, and access reports.
Children’s Incident Tracking and Reporting System
Children’s Incident Tracking and Reporting (CITR) is a secure data collection, tracking, and reporting aid to support local, regional, and state staff to work as a team to identify, track, and report incidents.
Children’s Functional Screen
The Functional Screen system is a web-based application used to collect information about an individual’s functional status (activities of daily living), health, and need for assistance to determine functional eligibility.
