  • Claim suspensions with error code 911
    ForwardHealth is aware of an issue in which claims are suspending with Explanation of Benefits (EOB) code 0000, "This claim/service is pending program review," and error code 911. This is the result of an internal system error. Providers should continue to submit claims as they normally would as we work to resolve the issue.

Children’s Long-Term Support (CLTS) Service Rates


This area is intended for qualified service providers currently approved to deliver services, supports, and goods to a CLTS participant. To access and download information on the Wisconsin Department of Health Services (DHS) established rates, see the list of covered CLTS services below. To access the entire schedule, select CLTS Waiver Program Rate Schedule (P-02184).

Some CLTS benefit categories and sub-categories are not included under the DHS established rates because they are paid at market rates (for example, housing support services, personal emergency response system, and respite camps). For more information see the CLTS Code Crosswalk (P-02283).
