Adult Long-Term Care Provider Enrollment Townhalls

The Wisconsin Department of Health Services (DHS) encourages all providers of home and community-based services delivered under one of Wisconsin’s adult long-term care (LTC) waiver programs to attend an adult LTC provider enrollment townhall. The townhalls are in preparation for the new Medicaid provider enrollment process, starting for adult LTC providers in September 2024. Adult LTC waiver programs include Family Care, Family Care Partnership, Program of All-Inclusive Care for the Elderly (PACE), and IRIS (Include, Respect, I Self-Direct).

The townhalls will cover topics related to the adult LTC provider enrollment project, the expected enrollment timeline, and information about provider training. The content of the townhalls will be the same, so you will only need to attend one session.

The townhalls will be held online through Zoom on:

You can submit questions ahead of time to the LTC provider enrollment email inbox at LTCProviderEnrollment@wisconsin.gov.

DHS encourages you to spread the word! If you know someone who would benefit from LTC provider enrollment information or is interested in the new enrollment process, please share these opportunities with them.

For more information about this project and to review the Q&A document, please visit the New Provider Enrollment System for Adult Long-Term Care page of the ForwardHealth Portal.

If you require American Sign Language, live captioning, or other assistance during the presentation, please email LTCProviderEnrollment@wisconsin.gov one week before the townhall you plan to attend. See the presentation in English (PDF), Hmong (PDF), and Spanish (PDF).
