The medical home is used as a model of care that is patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety. Currently, Wisconsin operates three medical home programs which serve the state's Medicaid managed care programs.

• Obstetric Medical Home Initiative (BadgerCare Plus and Medicaid SSI HMOs only)

  Initiated in January 2011, the Obstetric Medical Home (OBMH) is part of DHS' long-standing efforts to improve birth outcomes and reduce birth disparities in Wisconsin. The initiative was implemented in Southeast Wisconsin beginning in 2011 and expanded to Dane County and Rock County in 2014. The OBMH provides comprehensive, coordinated prenatal and postpartum care to BadgerCare Plus and Medicaid SSI HMO members who have been identified as high-risk for a poor-birth outcome. Care coordination is a key component, as is addressing psychosocial issues, (e.g., domestic violence, unstable living conditions, inadequate support system, etc.) and member engagement in her own care. • HIV/AIDS Health Home

  Effective with the 2016-2017 BadgerCare Plus and Medicaid SSI HMO contract, HMO members with HIV or AIDS are eligible to receive care through a HIV/AIDS Health Home program. This health home helps to ensure that all members diagnosed with HIV or AIDS have access to appropriate specialists and Medicaid-covered services. Wisconsin’s designated health home provider, AIDS Resource Center of Wisconsin (ARCW), provides services to eligible HMO members and has clinic locations in Dane, Kenosha, Brown, and Milwaukee counties. The HIV/AIDS Health Home is authorized under the Affordable Care Act of 2010, Section 2703.

• Care4Kids Foster Care Medical Home

  Care4Kids is a Medicaid program developed by the Department of Children and Families and the Department of Health Services that began on January 1, 2014 in the following six southeast counties: Kenosha, Milwaukee, Ozaukee, Racine, Waukesha, and Washington. Care4Kids is currently administered in partnership with Children's Hospital of Wisconsin and Children's Community Health Plan. The Care4Kids program provides comprehensive, coordinated care for children in out-of-home care that reflects trauma-informed principles and recognizes the unique needs of these children.
