This November, the Wisconsin Well Woman Program (WWWP) will release revised versions of the following three reporting forms. Providers must complete the most recent versions of these forms when submitting claims for reimbursement:
The submission process for these forms will not be affected if they were submitted prior to November 13, 2023.
On and after November 13, 2023, the previous versions of these forms (dated 10/2008) will no longer be available.
