Due to manufacturer terminations in the federal Medicaid Drug Rebate Program (MDRP), ForwardHealth will no longer cover several drug products effective October 1, 2025. This includes drugs covered in Preferred Drug List (PDL) classes and drugs covered outside of the PDL, which includes some brand medically necessary drugs.

Providers may refer to the Numeric Listing of Manufacturers That Have Signed Rebate Agreements data table on the Pharmacy Resources page of the ForwardHealth Portal for a complete list of impacted manufacturer labeler codes.

ForwardHealth will end coverage and remove PDL drugs associated with these manufacturers from the October 1, 2025, Preferred Drug List Quick Reference. The affected drugs in the PDL are temporarily listed on the last page of the Preferred Drug List Quick Reference data table on the Pharmacy Resources Portal page.

Pharmacy providers should work with a member’s prescriber to transition them to covered therapeutic options.

One of the manufacturers ending their participation in the MDRP is Bausch Health. After October 1, 2025, providers may refer to www.bauschhealthpap.com or contact Bausch Health by phone at 1-833-862-8727 to find out what noncovered drugs produced by Bausch Health may be available for Wisconsin Medicaid members through the Bausch Health Patient Assistance Program.
