Certain Dental Claims Will Reprocess With New Crown Rates
Reimbursement rates for dental procedure codes D2740 (Crown—porcelain/ceramic) and D2751 (Crown—porcelain fused to predominantly base metal) are now available on the ForwardHealth interactive maximum allowable fee schedules.
Suspended and denied claims for D2740 and/or D2751 with dates of service (DOS) on or after April 1, 2025, will be automatically reprocessed after the system has the reimbursement rate in place. This includes claims for DOS past the timely filing requirement.
Claims may still deny if they include any claim errors.
Providers should monitor their Remittance Advices or secure ForwardHealth Provider Portal account for reimbursement of the reprocessed claims. Providers may contact their field representative if they have any questions. Refer to the Assigned Professional Field Representatives by County map for contact information.
