	This search tool is moving! The Department of Health Services (DHS) is proud to announce the Wisconsin Provider Finder, a one-stop-shop for finding both health care and residential providers and facilities. The tool will be available on the DHS website in spring 2026. It lists Medicaid providers from the current Find Care tool, and Division of Quality Assurance (DQA)-licensed or certified facilities from the current DQA Provider Search tool. The new search experience has more filtering options, has a map and list view, and is usable on all devices. This page will redirect you to the new tool once it launches.
