The health and residential care provider information contained in Provider Search originates from an application hosted by the Centers for Medicare and Medicaid Services (CMS). This CMS-maintained application is being replaced with a new application. Some provider types have already migrated to the new application.
In order to present consumers with current provider information, Provider Search must be updated to use the new application. During this transition period, information regarding Ambulatory Surgical Center, Home Health Agency, and Hospice provider types may not be up to date on this website.
