ForwardHealth now requires providers to have an audit address on file. A new audit address panel has been added to the ForwardHealth Portal that allows providers to indicate a specific address to which all audit-related correspondence will be sent, ensuring that time-sensitive audit materials are received in a timely manner.

Currently enrolled providers are required to review their audit address as soon as possible, but no later than May 8, 2017, and revise it as necessary using the demographic maintenance tool on their secure Provider Portal account. ForwardHealth has automatically entered the provider’s practice location address into the audit address panel. This address will remain on file until it is changed by the provider. Refer to the ForwardHealth Portal Demographic Maintenance Tool User Guide more information about using the tool.

Providers will be prompted to enter or review their audit address any time they enroll, re-enroll, or complete revalidation on the Portal.
