DQA-Division of Quality Assurance Provider - Search The Provider Search application provides an easy and fast method for consumers to find health and residential care providers in Wisconsin that are regulated by the Division of Quality Assurance.

Online Help is displayed in a pop-up box if a user clicks on any label in the Provider Search application. The table below is a list of the label/field names containing Online Help information.

Adult Day Care A day program that provides the elderly and other adults with services when their caregivers are at work or need relief. Only those adult day care programs seeking to serve one or more participants with Waiver fundings are required to be certified.
Adult Family Home A place where 3 or 4 adults who are not related to the operator reside and receive care, treatment or services that are above the level of room and board and that may include up to 7 hours per week of nursing care per resident.
Advanced Aged Allows the user to search for ADC, AFH, or CBRF providers that serve the Advanced Aged client groups.
Alcohol/Drug Dependent Allows the user to search for ADC, AFH, or CBRF providers that serve the Alcohol/Drug Dependent client group.
Alcohol/Mental Health A wide range of programs for persons affected by alcohol, drugs or mental illness. These programs include detox centers, day treatment, inpatient/outpatient facilities, residential programs, intervention/prevention efforts, inpatient/outpatient facilities, intervention/prevention efforts, and employment services.
Ambulatory Surgical Center A place that provides day surgery services to persons who need less than 24-hour nursing/medical care.
Assisted Living Facilities-All Facilities for persons who need some level of care monitoring services but choose to live in a setting without 24-hour access to nursing services. Facilities types include Adult Day Care, Adult Family Home, Community Based Resident Facility, and Residential Care Apartment Complex.
City Select a Wisconsin city from the drop down to search for providers within that city. The city drop down is only populated after a valid Wisconsin county has been selected.
Client Groups Served ? Assisted Living Facilities Only Allows the user to select specific client groups served to located DQA regulated facilities that serve these client groups. A search for client groups served requires a user to search by Adult Day Care Facilities, Adult Family Homes or Community Based Residential Facilities facility types.
Clinical Laboratory A facility that analyzes material from the human body to diagnose, prevent, or treat a disease or for assessment of health.
Community Based Residential Facility A place where 5 or more unrelated people live together in a community setting. Services provided include room and board, supervision, support services, and may include up to 3 hours of nursing care per week.
Comprehensive Outpatient Rehabilitation A place that provides rehabilitation services on an outpatient basis.
Correctional Clients Allows the user to search for ADC, AFH, or CBRF providers that serve the Correctional Clients client group.
County Select a Wisconsin county from the drop down to search for providers within that county.
Developmentally Disabled Allows the user to search for ADC, AFH, or CBRF providers that serve the Developmentally Disabled client group.
Distance Select a radius of miles from the drop down to locate providers within that distance of the entered zip code. A user must enter a zip code in conjunction with entering a distance radius.
Emotionally Disturbed/Mental Illness Allows the user to search for ADC, AFH, or CBRF providers that serve the Emotionally Disturbed/Mental Illness client group.
End Stage Renal Dialysis A treatment provided two or three times a week that removes wastes from the blood that the kidneys are unable to remove.
FDD ICF-IID Facilities serving people with Developmental Disabilities (FDD), also known as Intermediate Care Facilities for Individuals who are Intellectually Disabled (ICF/IID): A residential facility for three or more unrelated persons with developmental disabilities.
Home Health An organization that provides part-time and intermittent skilled nursing and other therapeutic services on a visiting basis to persons in their homes.
Hospice An agency that provides care to terminally ill persons who have a life expectancy of 6-12 months. This care can be provided in the person's home or in another setting (e.g., nursing home, community based residential facility, etc.)
Hospital A place that provides 24-hour nursing/medical care to diagnose and treat short-term illnesses and/or injuries.
In Home Care-All Provider types that usually provide services in a person?s home. Provider types include Home Health, Hospice, and Personal Care Agency.
Include Closed Faclities By selecting this option, the search results will include facilities that have closed within the last 2 years.
Irreversible Dementia/Alzheimers Allows the user to search for ADC, AFH, or CBRF providers that serve the Irreversible Dementia/Alzheimer?s client group.
Non-Long Term Care-All The following provider types are identified as Non-Long Term Care: Ambulatory Surgical Center, End State Renal Dialysis, Hospital, Clinical Laboratory, Outpatient Rehabilitation, Outpatient Therapy, and Rural Health Clinic.
Nursing Home A place where 5 or more persons who are not related to the operator or administrator reside, receive care or treatment and, because of their mental or physical condition, require access to 24-hour nursing services, including limited nursing care, intermediate level nursing care and skilled nursing services.
Outpatient Physical Therapy/Speech Pathology A place that provides therapy services on an outpatient basis.
Personal Care Agency For Medical Assistance certification, a personal care provider shall be a freestanding personal care agency, a home health agency licensed under s. 50.49, Stats., and ch. DHS 133, a county department established under s. 46.215, 46.22 or 46.23, Stats., a county department established under s. 51.42 or 51.437, Stats., which has the lead responsibility in the county for administering the community options program under s. 46.27, Stats., an independent living center as defined in s. 46.96 (1) (ah), Stats., or a federally recognized American Indian tribe or band in Wisconsin.
Physically Disabled Allows the user to search for ADC, AFH, or CBRF providers that serve the Physically Disabled client group.
Pregnant Women/Counseling Allows the user to search for ADC, AFH, or CBRF providers that serve the Pregnant Women/Counseling client group.
Provider Name Enter at least 4 characters of the provider's name that you wish to search for.
Provider/Facility Type Allows the user to select provider/facility types regulated by the Wisconsin Division of Quality Assurance to search for these providers.
Public Funding Allows the user to search ADC, AFH, or CBRF providers that serve clients with Public Funding.
Records To Display Select the number of search result records to display per page from the drop down.
Residential Care Apartment Complex Independent apartment units in which the following services are provided: room and board, up to 28 hours per week of supportive care, personal care, and nursing services.
Residential Care-All Provider types providing care at a facility: Resident Care provider types include Assisted Living Facilities (Adult Day Care, Adult Family Home, Community Based Resident Facility, and Residential Care Apartment Complex), Nursing Homes, and Facilities serving people with Developmental Disabilities (FDD), also known as Intermediate Care Facilities for Individuals who are Intellectually Disabled - ICF/IIDs
Rural Health Clinic A clinic that is located in a rural area with a shortage of health care professionals. Services offered are similar to those received in a doctor's office and are often provided by a nurse practitioner or a physician assistant.
Search Criteria Allows the user to enter desired search criteria to locate providers regulated by the Wisconsin Division of Quality Assurance. To perform a search, a user must enter at least county, city, or ZIP code.
Sort Order Select a sort order from the drop down for the display of the search results. Distance can only be selected when a zip code and a proximity radius are entered.
State Select a State from the drop down to search for providers within that state. Only providers regulated by the WI Division of Quality Assurance will be included.
Terminally Ill Allows the user to search for ADC, AFH, or CBRF providers that serve the Terminally Ill client group.
Traumatic Brain Injury Allows the user to search for ADC, AFH, or CBRF providers that serve the Traumatic Brain Injury client group.
Veterans Allows the user to search for ADC, AFH, or CBRF providers that serve the Veterans client group.
ZIP Code Enter a zip code to search for providers located in that zip code.
