Account Setup This is where the Provider, MCO, and Manufacturer administrators will setup their account after receiving their PIN Letter.
Field Descriptions

Cancel Click on Cancel to clear the detail fields without saving the information.
Confirm E-mail Retype the email address that you entered above.
Confirm Password Re-enter the account user's password.
Contact First Name Enter the first name of the contact person for the account user.
Contact Last Name Enter the last name of the contact person of the account user.
E-mail Enter the email address of the account user.
First Answer Enter the answer to the first question that you submitted previously.
First Security Question Enter a security question for your account that only you can answer. The security question can be anything you want.
Login ID Enter your login identification number.
Password Create a password to access this site. The password must contain one uppercase letter and one number.
Personal Identification Number Enter your personal identification number (PIN).
Second Answer Enter the answer to the second question that you submitted previously.
Second Security Question Enter a security question for your account that only you can answer. The security question can be anything you want.
Security and Confidentiality Review the security agreement that displays in this field.
Setup Account Click on the Setup Account button to verify login information and to display the user profile panel.
Submit Click on the Submit button to submit the user profile and return to the home screen.
Telephone Number Enter the contact person's telephone number.
Telephone Number Ext Enter the contact telephone number extension of the account user.
User Name Enter the user name from the Personal Identification Number (PIN) letter that you received.
Wisconsin Check the box to show that you agree with the security agreement.
