Contact Information The Contact Information page is made up of the following sections Programs: Fields that allow you to pick from a list of pre-defined Programs. User Type: Fields that allow you to indicate which type of user they are. Question: Fields based on the question being asked to assist the help desk to answer your question. Contact Method: Fields that allow you to indicate how you would like to receive the response to your question. Methods to receive your response are by phone, email, regular mail, or no response needed. Confirmation: this is a message displaying the tracking number.

Confirm E-Mail You must re-type the email address to confirm it.
Do you require a response? Select Yes if a response is required or No if a response is not required.
E-mail Address Enter an email address where a response can be sent.
Ext. Enter a phone number +5 digit extension where you can be reached.
First Name Enter the first name of the user.
How can we help you? Enter any comments.
Last Name Enter the last name of the user
Member ID Enter the identification number (ID) of the member.
PHI Disclaimer Information that is being sent may be PHI, and it needs to be protected. It is protected after submission, as well.
Provider ID Enter the provider Id
Select User Type Select the user type that best represents you from the following values: Provider, Member, Partner, Person not yet a Member, or Other.
Select a Program Select a program from the list of programs provided.
Telephone Number Enter a phone number +5 digit extension where you can be reached.
Trading Partner ID Enter the identification number (ID) of the trading partner.
